Person insured under US NetCare plans may choose to be treated within or outside
of the provider network (PPO). Each PPO network consists of hospitals, doctors and
other health care providers organized for the purpose of delivering quality health
care at affordable rates. Reimbursement rates will vary according to the source
of care and as described under the Summary Schedule of Benefits of each plan.
Using a network health care provider will save you money by getting negotiated reduced
rates for service. It also provides a better channel of communication between your
insurance company and the service provider for authorization of coverage and claims
processing. Make sure to carry your medical insurance card on you at all times.

the Beech Street Network. Reimbursement rates will vary according to the source

The completed claim form, all itemized bills, statements and receipts must be sent
to the claims administrator no more than 90 days after a covered loss occurs or
end, or as soon after that as is reasonably possible.
